Consolidated information for the assessment
Each organization keeps its own folders. A phone that calls two organizations has two folders. A known number is not filed as the same person until a clinician says so.
01
What the folder holds
Name, once a clinician has typed it. Year of birth, language, allergies, conditions, a usual place of care, and a safety note for the next clinician. The pathway never asks the caller for a name. Until then the folder is known by the number.
02
The same-person check
A number never seen here starts a new folder. A number already on file waits. The clinician confirms, or opens a second folder so two people who share a phone are not mixed. On a red call the question is on screen only.
03
What leaves the building
Earlier contacts travel to the emergency department, the crew, or the person’s own summary only after that confirmation. A number that merely matches does not travel.
04
The person’s own door
My Health Folder is a sign-in: username, password, and a one-time code. No links. The organization decides whether that door is open. A parent can hold a child’s folder. A folder from another organization cannot be attached.
